Responsibilities:
  • Perform thorough evaluations, assessments, and treatments for patients recovering from illness, injury, or surgery
  • Develop and implement individualized care plans and therapeutic interventions to improve patient mobility and function
  • Collaborate closely with physicians, nurses, and other rehabilitation team members
  • Educate patients and families on therapy plans, mobility techniques, and home exercise programs
  • Accurately complete all required documentation and patient records in a timely manner
  • Ensure care and services meet regulatory, facility, and professional standards
  • Participate in quality improvement initiatives and staff in-services
  • Physical Therapist (PT)
Qualifications:
  • Active, current, unrestricted Physical Therapist (PT) license in the state of practice
  • Graduate of an accredited Physical Therapy program (CAPTE accredited)
  • Experience in skilled nursing, rehabilitation, or post-acute care preferred
  • Strong clinical, communication, and organizational skills
