Schedule:
Monday – Friday 8:00AM-4:00PM (40 hours)
Summary:
The Case Manager – Pathfinder Services is responsible for supporting individuals in obtaining and maintaining housing, maximizing benefits, working with external agencies, establishing community relationships, and accessing internal resources as needed.
Essential Duties & Responsibilities:
Develop and maintain positive relationships with local service agencies, providers, realtors, and community landlords.
Work in conjunction with Program Manager and DMH on the identification and engagement of new referrals as well as the completion of the critical needs assessment.
Complete biopsychosocial assessment within seven days of entry into the Safe Haven program.
In conjunction with the participant, complete the community service plan within 2 weeks of entry into the Safe Haven.
Conduct individual and group sessions focused on ensuring housing stabilization, housing challenges, recovery support, and other topics.
Work with individuals on maximizing benefits and income including entitlements, work opportunities, and insurance management.
Meet with participants weekly for scheduled case management sessions and document using the electronic health record.
Facilitate referrals for medical, dental, and mental health services.
Maintain required documentation according to DMH regulatory standards.
Maintain core training certifications including FA/CPR, PABC and MAP and all other mandatory training courses based on Bridgewell’s policies and procedures.
Maintain other additional required training and certifications required by the program.
Develop positive relationships with DMH, providers, guardians, family members and other stakeholders.

Required Education/Experience:
High School diploma/GED with a minimum of 2 years of successful employment within a housing and/or benefits maximization focused program is required.
Associate’s degree or higher is preferred.
Experience in substance use disorder and mental health services is preferred.
Required Skills/Knowledge:
Knowledge of housing services, challenges, and resources within demographic area
Knowledge of Massachusetts entitlements, housing subsidies, vocational opportunities, and State insurance programs.
Proficient in planning, prioritizing, and executing assigned tasks efficiently and on time.
Communicates effectively, both verbally and in writing.
Actively listens and demonstrates empathy and understanding.
Sets clear boundaries while showing compassion and respect.
Must have a current US driver’s license and one year of driving experience.
Computer literacy to include the ability to use proficiently Microsoft products and web-based applications used by the agency.
Bilingual/bicultural preferred especially in Spanish or Khmer.
Physical Demands:
The physical demands of staff working in residential programs assisting people with their care needs are varied and require the employee to be able to maneuver people of varying weights, and provide lifting, balance support and physical intervention in several scenarios. Described below are essential requirements of staff to ensure the safety of employees and people served alike. Reasonable accommodations may be made to enable a person with a disability to perform the essential functions; however, approval will be based on the needs of the persons served and consideration of safety factors related to any program or position within a program. While performing the duties of this job, the employee is required to be able to: Climb stairs, physically assist persons served, often weighing more than 200 pounds, with movement, bathroom routines including toileting, assisting with entry and exit of a tub or shower, getting into and out of wheelchairs or dealing with assists that include physical, behavioral and medical interventions.
Additional lifting or moving may be required as part of activities in the daily living of people in our services.
