$25.06 – $32.58 / hour Summary Responsible for increasing utilization of Lee Healths products and services. Partners with the acute care hospitals, Skilled Nursing facilities, the Rehabilitation Hospital and Home Health to navigate patients throughout Lee Healths Post-Acute Continuum. Will identify process improvement opportunities and at times lead the team in implementing them. Facilitates a timely, integrated and coordinated approach to the care of the patients. Effectively performs in the role of facilitator, advocate, consultant and/or researcher. Responsible for being the resource and solution to all patients post-acute needs. Meets with patients at the bedside who are referred to Lee Health Skilled Nursing Services, Lee Health Home Health or the Rehabilitation Hospital and providing information on the continuity of care services provided by Lee Healths Post-Acute continuum. Secures physician to sign orders as appropriate. This position must be flexible to cover any acute care facility, skilled nursing facilities, physician offices, the central office and other locations that may need assistance. Assists in care opportunity development from hospitals, skilled nursing care centers and assisted living facilities. Also supports the Physician Relations Department with various duties such as printing care opportunity reports and participating in community education and outreach. Will assist in developing and supporting referral sources/customer relationships. Will participate in regular staff and operations meetings to provide service line education and updates.

Requirements Education:

Bachelors or Associates degree.

Experience:

Knowledge of the healthcare arena, experience in relationship management and business development in the hospital based setting preferred, but not required. Candidates holding an Associates Degree, extensive business development experience, or significant healthcare industry experience is required.

Excellent communication, interpersonal, and customer relation skills required, Ability to learn, analyze, understand and convey healthcare terminology is required. Effective organizational and communication skills, Leadership, self-motivation and initiative, results oriented with demonstrated time management skills.
