Hours:

Monday – Friday 8:00am-5:00pm -(40 hours/week). Other hours and/or days as needed for program activities.

Qualifications:

A Bachelor’s or Associate degree in the Health Sciences is preferred, experience in data entry and/or consumer relations, and knowledge of Electronic Medical Record. Other college degrees will be considered based upon the experience of the applicant. Job duties include but are not limited to: Provide reports to the Program Coordinator regarding issues and concerns related to data entry. Monitor clinical progress while creating and maintaining individual medical encounters for clients seen at Low Country Health Care System sites. Document all activities and services provided for patients in Provide Enterprise. Flag patient charts to let providers know what labs are due Make and follow-up on all dental referrals. Provide troubleshooting assistance to Program Coordinator for entire EIS program. Provide minimal case management activities in absence of medical case managers to ensure continuity of care. Pickup/deliver medicine and document activity in Provide Enterprise Transport patients to appointments Provide referrals to Outreach when clients become at risk of falling out of care Import labs into Provide Enterprise Key in all paid prescriptions, kept appointments, dental appointments, vouchers, and transportation into Provide Enterprise. Check charts daily to ensure all services received are properly documented in Provide Enterprise. Run monthly reports for EIS staff. Monitor clients who tested positive for a STD/STI Participate in CQI activities Assist with LCHCS reporting needs Manage charges and CAPS for Ryan White clients on Sliding Fee Scales C, D, E Attend training sessions for reporting Perform any other duties as assigned by your supervisor •Salary scales are a range of pay and offers are made to applicants based upon the requirements of the particular position.
