Position Title:
Data Coordinator – Hospital Readmissions and Transitions of Care (TOC)

Position Summary:
The Data Coordinator will be responsible for collecting, analyzing, and reporting data related to hospital readmissions, transitions of care (TOC), and other related quality metrics. This individual will work closely with clinical teams, care coordinators, and leadership to track outcomes, identify trends, and provide actionable insights to support performance improvement initiatives and reduce avoidable readmissions.
Key Responsibilities:
Collect and compile data from various internal systems (EHR, case management, discharge planning, follow-up systems) relevant to: 30-day readmissions Transitions of Care (e.g., follow-up appointments, medication reconciliation, post-discharge calls) ED revisit rates Social determinants of health impacting discharge outcomes Quality metrics such as HEDIS or CMS indicators Maintain dashboards and trackers that clearly communicate progress and trends to hospital leadership and stakeholders. Develop and distribute regular reports (weekly, monthly, quarterly) to leadership, department managers, and front-line teams. Collaborate with clinical and operational teams to ensure data accuracy and timely reporting. Assist in the design and implementation of quality improvement projects focused on TOC and readmission reduction. Translate complex data into user-friendly formats (charts, graphs, infographics, presentations) to support decision-making. Monitor performance against established benchmarks and KPIs, flagging outliers and trends for further review.
Qualifications:
High School Diploma or equivalent 2+ years of experience in a healthcare setting working with data analytics, case management data, or quality improvement programs. Proficiency in Excel, PowerPoint, and data visualization tools. Experience working with EHRs (e.g., Epic, Cerner, Meditech) and extracting data from clinical systems. Excellent written and verbal communication skills, including the ability to present data to non-technical audiences. Detail-oriented, highly organized, and able to manage multiple projects under deadlines.
Preferred Skills:
Familiarity with population health platforms or care coordination software. Knowledge of risk stratification models and predictive analytics tools in healthcare.
