Full Job Description General Description:

Obtains and enters Ryan White client intake and recertification information into CAREWare. Demonstrates an extensive working knowledge of Ryan White program reporting requirements and familiarity with all applications associated with managing CAREWare data reporting. Provides timely data reports for internal Quality Management projects and meetings, as well as data collection and reporting of units of service and aggregate outcomes provided for Part A funded services.

Duties and Responsibilities:

Interviews clients at intake and recertification for Ryan White eligibility, prepares appropriate documentation, and initiates and updates CAREWare record. Receives and scans required documents for Ryan White eligibility into the electronic health record and CAREWare at intake and recertification, as well as data on units of services provided to RW clients into CAREWare. Assists with the promotion of medical home, team-based care by working with internal/external partners to help patients overcome barriers within the healthcare system, such as helping arrange patient transportation as needed. Develop/prepare a data system/means (e.g., data export) to collect and manage client-level and aggregate services and outcomes data in CAREWare. Helps train other Ryan White staff on CAREWare utilization/services data and reporting. Manage data systems for Ryan White Program to meet planning, grant reporting, performance and quality management requirements. Work closely with multi-disciplinary team to ensure complete review of the data, as well as provide a thorough understanding of data management processes and issues that may arise in analysis and reporting. Ensure required submitted data reporting is complete and correct and communicate findings effectively to RW team members. Analyze client-level utilization data (CLD) for accuracy and consistency with quarterly reports. Provide technical assistance to team members to support their appropriate documentation of services and outcomes. Effectively communicate data to the Ryan White team members and providers. Provides, presents, and analyzes data for Continuous Quality Improvement activities, Patient Outreach and Retention Reporting, Screening, Brief Intervention, & referral to treatment quarterly reporting, or as otherwise indicated. Prepare service-specific custom data reports as needed by the Ryan White Program Manager, Quality Manager, fiscal staff, and Fulton County RW Program staff. Develop and deliver semi-annual Service Area Reviews that incorporate results of client-level outcomes evaluation and utilization data for use by SMC in its prioritization and allocation process. Compare CLD to annual HIV epidemiology report from the Georgia Department of Health and analyze for utilization trends. Assist in preparation of the annual Part A grant applications and in meeting Ryan White grant conditions of award. Upholds guidelines set forth in clinic/organization, including maintenance of patient confidentiality. Participates in meetings and committees as necessary. Performs other duties as necessary to cover Ryan White program needs and meet deadlines. Completes special projects as requested.

Knowledge, Skills and Abilities:

1. Knowledge of CAREWare data system, and ability to translate services recorded within electronic health records into required data elements and services information into CAREWare . 2. Ability to communicate with sensitivity to needs and concerns of patients living with HIV. 3. Excellent documentation skills, with ability to check data for errors and assure accuracy within required reports. 4. Ability to provide technical guidance and direction to others in support of accurate data utilization and reporting. 5. Ability to ensure adherence to all applicable policies and procedures. 6. Knowledge of medical terminology and Ryan White service category-related reporting.

Minimum Qualifications:

Associate’s Degree in computing or computer support and five or more years of experience completing data input for CAREWare database and electronic health record systems. Requires sitting for extended periods of time, as well as some bending, stooping, and stretching.

Typical Demands:

Requires manual dexterity and eye‐hand coordination sufficient to operate a keyboard, photocopies, telephone, calculator, and other office equipment. Requires normal range of vision and hearing. May require sitting for prolonged periods of time during patient appointments and documentation. Involves physically assisting patients, occasionally lifting and carrying items weighing up to 50 pounds.

Typical Physical Demands & Working Conditions:

Administrative and medical office environment with potential for exposure to communicable diseases, bodily fluids, toxic and other substances common to clinic environment.
