MINIMUM QUALIFICATIONS

Bachelor’s Degree in Information Systems, Business Analytics, Economics, Social Work, or other applicable degrees considered based on the overall alignment of experience. Knowledge of Word, Excel, and other computer programs Annual Physical (required within 1 month of hire) Clean, Valid NYS Driver’s License (copy required upon hire) Ability to work with a diverse population presenting a variety of cultural, socioeconomic, developmental, and sexual identities. Must have the ability and willingness to establish and foster healthy and supportive relationships that acknowledge individual identities and encourage personal growth.

PREFERRED QUALIFICATIONS 1-2

years of experience in a Residential Treatment Center or Facility Previous experience with NYS Connections Previous experience with the Human Services Learning Center (HSLC) Previous computer spreadsheet experience Training experience

OBJECTIVES OF POSITION

Provide Connections system support and training to The William George Agency.

DUTIES AND RESPONSIBILITIES

Provide training in Connections systems use for all new Cottage Professional Staff. Provide ongoing Connections training for all Cottage Professional Staff and Administrators when appropriate. Open and close all Agency cases within Connections and work with Case Managers (County Caseworkers) to enhance the overall Connections process. Track all Connections cases for the Agency, verifying case initiation dates to ensure accurate scheduling of planning conferences as well as general troubleshooting. Complete Family Services Intake Stage for all applicable OCFS cases upon admission. Re-assign all Connections cases that may be transferred within the Agency. Troubleshoot, consult, and provide an immediate resource for Connections operations for all Behavioral Health Clinicians, Family Workers, ILS Specialists, Case Planners, and Administrators. Attend all Connections trainings and related meetings to relay all information back to Agency staff with Connections obligations. Assign Clinic staff to all cases for medical data input relative to Connections. Conduct Case Record Reviews on a monthly/quarterly basis relative to case notes, etc., and report such findings to your immediate Supervisor and the appropriate Residential Supervisor. Develop and perform comparative analysis opportunities related to trends in case management to include family contacts, FASP completions, and other case management areas. Report trends in the area of case file management to the QA Supervisor. Work with client stakeholders, Behavioral Health Clinicians, Family Workers, Independent Living Specialists, Case Planners, and Administrators to gather and receive data related to case management and identify areas requiring improvement for inclusion in the PQI Committee review process. Review all County Contracts relative to Connections requirements and ensure that the Agency meets all obligations related to such contracts. Provide administrative support for Agency staff as a training coordinator for HSLC. Conduct audits through the Electronic Health Record (EHR) system to ensure accuracy, compliance, and quality of documentation. Implement and monitor policies and procedures related to case management systems, ensuring alignment with OCFS requirements and Agency goals. Conduct research to support program development, quality assurance, and performance improvement initiatives. Utilize data modeling and project management tools to support planning, forecasting, and process improvements. Create, manage, and maintain dashboards for real-time monitoring of key metrics related to case management, compliance, and outcomes. Adhere to all rules, regulations, policies, and philosophy of The William George Agency and the Office of Children and Family Services. Perform other duties as assigned by the Director of Quality Assurance.
