DSP – Community Hab 2.7 2.7 out of 5 stars 175 Humphrey Street, North Tonawanda, NY 14120 We have Full-time (40 hours), Part-time and Relief positions available for this role. Direct Support Professionals (DSPs) provide a wide variety of person-centered-supports and services to people with developmental disabilities. DSPs empower the people they work for by providing education, experience and exposure so they can make their own informed choices. DSPs model, teach and advocate for people with developmental disabilities in order to support them to achieve their highest level of independent living, enable their fullest participation in society, ensure their civil and human rights are protected and promote their ability to be productive, contributing members of the community. The supports and services performed depend upon the habilitation plan.

POSITION RESPONSIBILITIES

Understands and follows the National Alliance of Direct Support Professionals (NADSP) code of ethics Maintains their first allegiance to the people they work for by demonstrating respect and supporting problem solving skills. Has solid knowledge and understanding of person centered planning and service delivery. Promote daily thinking and problem-solving skills of the person served, using active listening and coaching skills in order to reduce their dependency on staff and increase their self-esteem. Encourages growth and recognizes autonomy while being attentive and energetic in reducing the risk of harm at all times by providing all necessary supports indicated in the IPOP. Assists in the development (up to and including attending ISP meetings), implementation and on-going evaluation of habilitative service plans that are based on the person’s preferences, needs and interests. Helps peoples to develop/maintain positive interpersonal relationships with their peers, with others in their families and with people in the community. Actively participates at staff meetings by communicating observations concerning progress and reaction to various therapy programs. Completes accurate documentation in Therap within required timeframes, based on the implementation of the habilitation plan. Provides assistance with navigating the community up to and including providing transportation to people’s homes when necessary. Completes DDP-2s, incident reports and other agency forms necessary for service delivery. Provides ongoing communication within and across departments to ensure proper service delivery is provided and maintains a minimum of one monthly contact with other service providers of all peoples on caseload, and documents these contacts in case notes. Develops solid working relationship with the individuals being served as well as any family members, care providers and other people important to the person being served. Supervises and assists individuals at volunteer sites. Purchases appropriate program supplies with the approval of the manager. Assists to coordinate, plan and conduct daily activities and/or secure participant volunteer placements if needed. Education and Experience High School Diploma/GED Skills, Knowledge and Abilities Good oral and written communication skills Must be able to read and write case notes as well as understand written directions. Ability to manage multiple tasks and cope with stressful situations and deadlines. Demonstrate the ability to exercise independent judgment and maintain concentration. Create and maintain positive interaction with others. Basic computer skills. Be authorized as an Agency Approved Driver• by Holding and maintaining a valid NYS Driver’s License. Maintaining an approved driving record according to Agency Policy and Procedure. Full-time and Part-time staff must have use of personal vehicle for work purposes; travel between work locations and transporting people served; not required for Relief staff. Full-time and Part-time staff must have proof of insurance being current as a driver on personal vehicle being used for work purposes; not required for Relief staff. Mileage reimbursement for use of personal vehicle is provided. Successful completion of Introductory Period Training Requirements Knowledge of developmental disabilities. CPR/First Aid certified. SCIP-Core certification Physical and Mental Requirements Seeing, hearing, walking, sitting, standing, driving, stooping, bending, carrying, stretching, and climbing stairs as well as the ability to perform all required movements required by the SCIP-R curriculum. Ability to lift up to fifty (50) pounds. Staying awake and alert while on duty. Working Conditions Will work at program locations and within the community. Schedule may include early mornings, evenings, and weekends; schedule is determined by needs of peoples. Local travel required Provide services to peoples in their homes as well as in the community. Float and Relief Staff will be required to work at various locations, as needed. Relief Staff must work a minimum of 12 hours per month. Training hours do not count towards work time for hours minimum. Community Services for Every1 has supported the people of our community for over 30 years.
