CORE JOB SUMMARY
The Outreach Driver identifies, engages, and transports individuals in need to HealthyMD clinics so they can receive HIV awareness and prevention education and to ensure they are provided STD, Hep C and HIV testing, counseling, and referrals.
CORE JOB FUNCTIONS
Engages in street and community outreach. Recruits’ participants for HIV/STD/Hep C testing, treatments, and counseling services. Engages street contacts in risk reduction discussions. Gives out
HIV/HEP C/STD
risk reduction information. Makes home visits and reaches out to persons/patients that present an adherence problem to treatment. Ensure data collection in a timely, accurate, and consistent manner. Distributes preventive materials including safe sex kits. Provide Outreach and Recruitment activities including but not limited to representing the program and the agency at community and committee meetings, providing educational workshops and social marketing activities.
