REPORTS TO
Administrator/ Transportation Supervisor Non-Exempt
PURPOSE OF POSITION
The primary purpose of your job position is to provide transportation safely and timely, per established policies and procedures, and as directed by your supervisor. ROLE and
RESPONSIBILITIES
Communicate with the dispatcher on Two-Way Radio Be able to operate a Hydraulic Lift. Report to Dispatcher and pick up patient schedule daily. Transport patients to and from doctors’ appointments. Inspect the van daily. (ex: check oil, tires, first aid kit & fire extinguisher) Ensure that seat belts secure all passengers. Other related duties and responsibilities may become necessary to ensure that the facility’s needs are met.

EDUCATION/EXPERIENCE/QUALIFICATIONS
Must be a minimum of 25 years of age. Must have CPR and First Aid License. Must have clean driving record with no more than 2 moving violations or 1 at fault accident in a 7-year period. Must be able to operate a tablet and complete computer trainings This position is highly physical, with frequent bending, pushing, and lifting of approximately 75 pounds or more multiple times a day in a vehicle.
