Mercy LIFE West Philadelphia, a member of Trinity Health PACE, is seeking a Driver (Paratransit Bus – Mini Bus) for its location in Philadelphia, Pennsylvania. The Program of All-Inclusive Care for the Elderly (PACE) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. PACE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee’s care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, PACE is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible. The Driver is responsible for transporting and/or assisting with transporting LIFE participants to and from the LIFE Center and to other appointments. Must have a valid Pennsylvania Driver’s License.
Experience:
Transportation and human services experience. Commercial Driving License preferred. Good driving record. CPR trained or willingness to be trained. One year of experience with a frail or elderly population
Possess:
Ability to communicate well in English, orally and in writing. Ability to interpret and follow oral and written instructions. Organizational skills Supervisory skills
Position Purpose:
The primary purpose of your job position is to provide safe and timely transportation of PACE participants, including transport meals, durable medical equipment, pharmacy, and any other transportation as deemed necessary. Responsible for overall cleanliness, safety of the vehicle assigned on a daily basis, and communicating any safety issues to the department leader.
Position Details:
This position will be housed primarily at the LIFE West Philadelphia.
What you will do:
Responsible for the PACE passengers during transport, providing care in a manner which ensures participant satisfaction. Follow assigned schedule, make adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE center, as well as communicating these changes to the department leader. Operate equipment safely and in compliance with the vehicle licensing regulations. Transport participants to and from medical/specialist appointments and activity outings as requested by the interdisciplinary team via the plan of care. Transport meals, durable medical equipment, pharmacy, and any other items as deemed necessary for the participants. Provide physical assistance to each participant when needed as he or she boards and debarks from the programs vehicles. Handle potential infectious fluids with appropriate biohazard precautions and practices Universal Precautions. Maintain the vehicle in a neat and clean condition, ensuring vehicle equipment is maintained and operating within the manufacturer’s recommendations; replacing supplies as needed.
Minimum Qualifications:
Must possess a comprehensive knowledge obtained through high school diploma or equivalent. Associate’s degree preferred. Valid driver’s license in good standing required. CDL license preferred but not required. Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
