The number of individuals in a workload depends on the number of hours worked. As part of this process the Community Habilitation Coordinator supervises Community Habilitation DSPs to carryout the service and oversees other pertinent activities such as individual attendance, vehicle mileage, petty cash, scheduling DSPs for trainings, maintaining pertinent records, etc.

Full-Time Exempt Work Shift:

5 days, 40 hours a week (must be flexible with adjusting hours to accommodate evening and weekend service delivery/be available for calls during afterhours)

Responsibilities :

• Plan for Community Habilitation supports for individuals (within established time frames) to include: Develop a functional assessment, On the Job Training and appropriate safeguard Write Community Habilitation Plans as directed in the Life Plan Develop Staff Actions that will support individuals to attain their valued outcomes Monitor the delivery of service at various times, days (including weekends), as defined by individual schedules, gather intervention data, and develop a monthly note relating to Community Habilitation outcomes•Oversee and observe Community Habilitation DSP's to ensure that quality services are delivered in accordance with the Staff Action Plan.
• Ensure that daily service interventions are completed in a timely and accurate manner by DSPs.
• Contact DSPs to make adjustments to individual interventions whenever problems are noted.
• Review intervention records on a daily basis within Electronic Health Records system to ensure DSPs are completing data entry contemporaneously.
• Attend Life Plan Meetings for individuals on caseload, report on progress and issues noted
• communicate with Care Managers and other team members
• Maintain a strong rapport with individuals and their families on the caseload to ensure quality services are provided.
• Communicate agency policies and procedures and ensure that they are adhered to by subordinate staff.
• Evaluate DSP work performance and institute effective progressive discipline if needed.
• Accomplish quality assurance checks of records in accordance with agency procedure and schedule
• Schedule DSPs for required trainings and ensure subordinate staff maintain compliance with various trainings assigned (Compliance, Choking Prevention, CPR, Electronic Visit Verification, etc.)
• Participate in Management and HIPAA Privacy Standards training courses
• Perform other related duties as assigned.

Requirements:

Bachelor's Degree in a Health or Human Services related field preferred A clean driving record. A reliable vehicle with valid insurance and registration At least two (2) years DSP experience working with individuals with developmental disabilities Ability to plan, implement and evaluate the success of interventions for people with developmental disabilities
